After finding feces on a lounge chair, nurse Didi receives conflicting orders from her co-workers; Dr. James has a public meltdown; the staff tries to determine a woman's identity.
After Beverly's reassignment to another floor, new supervising nurse Patsy De La Serda arrives at the ward and immediately clashes with the other staff members; Jenna's position as the ward's director of medicine is made permanent, much to her dismay; Varla Pounder, a patient suffering from bipolar disorder, causes trouble for the staff.
Patsy files a grievance against Didi for remarks he deems inappropriate - Dr. James decides to go to a convention in Cleveland - a patient is caught in a romantic situation with her gentleman caller - Dawn gets evaluated with no exceptions.
A homeless patient is "dumped" on the ward; unhappy with Patsy's new campaign, Jenna attempts to replace it with an aerobic fitness program; Phyllis Marmatan, the daughter of a patient with cancer, discovers that her mother's chemotherapy has been discontinued; Dawn and Patsy's relationship trouble continues.
As Didi attempts to tackle the night shift at the ward alone, Jenna stops by with her husband Richard in order to assist him with an ingrown toenail; after calling the nurse agency for assistance, Didi is surprised to discover that the agency has sent Dawn, who is drunk after a fight with Patsy; Jenna takes an interest in a Swedish patient named Ingrid Larsen, who demonstrates that she is much fitter than her age suggests.
Phyllis gets the runaround as her mother's condition vacillates. Jenna tries to leverage her Cleveland opportunity into concessions from Dr. Stickley. Didi grows impatient when a debt is unpaid. An equipment malfunction injures a patient; the staff perform their caregiver concert.
Dr. Jenna James' plethora of research projects threatens to take over the ward. A frustrated Didi asks for a raise; Patsy's "Lean & Green" initiative causes problems; Dawn gets a surprising answer to what's been ailing her. Jenna receives unwelcome news that leads her to make an unholy alliance.
After discovering that she is pregnant, Dawn tries to find a way to tell Patsy; Jenna installs a new hospice care program into the ward; the staff struggle with assisting Dottie Levy, a patient suffering from severe alcoholism; while attempting to investigate her union benefits regarding her pregnancy, Dawn accidentally kick-starts an investigation into the ward's possibly toxic hand sanitizer; The daughter-in-law of a patient admitted for heart surgery is told by Jenna that her mother-in-law's state of health may be worse than they thought; Patsy has a medical emergency on the ward.
A server crash forces the staff to go "old school" - with disastrous results. Didi steps on Dr. Stickley's toes when making a decision about Arlene's mother-in-law, while Dr. James has issues getting through to a deaf patient. Dawn buys a cello, and learns that her life-changing news may not be what it seems.
The staff finds itself in an awkward situation when one of Jenna's "adult" study photos mistakenly makes its way into the Christmas card competition. Didi is tempted by a visiting doctor. A patient with terminal cancer proposes a "final solution" to a startled Jenna. Patsy and Dawn's relationship comes to a head.
Didi struggles to deal with her younger sister Sherrie, who's filling in at Billy Barnes and cozying up to Dawn. Jenna gets a new look and encounters resistance to her research methods. Patsy lets an award go to his head. The staff tries to balance patient flow and has issues discharging a patient they'd hoped never to see again.
As Dr. Jenna James looks to move past her hospice scandal, she finds her job threatened by a handsome new doctor and takes immediate action. Didi is inspired to fight for her union rights, while cutbacks force Patsy to return to floor duty. Dawn questions her marriage and later gets distressing news.
The staff faces an infectious disease crisis. Didi's family drama spills into work, in the person of Yvette, her sister-in-law. Dr. James reaches out to a pharmaceutical company and finds herself affected by a patient with a complicated past. A worn out Dawn is revived by Patsy's unpredictable advances.
While preparing for a conference, Dr. James is caught off guard by workplace politics. A felon with cancer and a history of daring escapes arrives at the ward. When his mother pays a visit, Patsy asks Dawn to pose as someone she's not. Didi's mother-in-law comes to Billy Barnes for treatment, sparking a family feud.
On the day of her "Sympoosium," Dr. James is plagued by unexpected complications. Patsy embraces his truth, and Didi finds herself caught between work and family. Dawn and Dr. James see uncanny visions of themselves in two British visitors, Dr. Pippa Moore and Sister Den Flixter.
The ward's fate is in doubt, so Didi tries to think of a way to save it, but is distracted by pressure from Dawn to be her health advocate. Meanwhile, the obstreperous Varla returns and clashes with her estranged daughter; Dr. James confesses her sins to her husband; and a memorial service has no shortage of drama.
Didi takes to the picket lines in a last ditch effort to save Billy Barnes; Jenna tries to get out of a hastily made offer; Patsy and Marguerite share secrets; Dawn inadvertently sets a chain of events in motion that seals the fate of the ward.
